Rep. Mike Michaud (D-Maine) has introduced the Chiropractic Care to All Veterans Act in the House of Representatives. The bill would require the Veterans Administration (VA) to have a chiropractic physician on staff at all major VA medical facilities by 2016. Osteoarthritis and Vitamin Deficiency

It is well known that calcium is crucial for preventing bone and joint problems, but new research suggests there may be another mineral we should consider: zinc. In particular, a study suggests that levels of zinc within the cartilage cells may help to explain why tissue destruction occurs in patients with osteoarthritis.

Arthritis is a leading cause of disability in the world, affecting 52.5 million adults in the US alone. Despite the prevalence of osteoarthritis, there are currently no cures to stop the progression of cartilage destruction that takes place in individuals with the condition. Researchers are still attempting to understand what happens at a molecular level to cause the tissue degradation.

Osteoarthritis results in the break down of cartilage between the bones, causing joint stiffness and swelling. Tissue destruction is caused by proteins called matrix-degrading enzymes, which are produced by cells within the cartilage. Matrix-degrading enzymes need zinc to survive, which led researchers to hypothesize that zinc levels play an important role in osteoarthritis.

Using lab mice, the researchers found that a protein called ZIP8 is responsible for transporting zinc within the cells, setting off a chain of events that eventually results in cartilage destruction. Their findings suggest that treatments to deplete zinc in the cartilage cells or inhibit this ZIP8 function may help to stop osteoarthritis. If the research is confirmed in future studies, keeping zinc levels in check could become an integral part of osteoarthritis treatment.

Many patients with osteoarthritis find that it can be successfully managed by a conservative, multimodal treatment, including exercise, nutrition, and chiropractic care. Research suggests that a combination of chiropractic and exercise can significantly ease symptoms in patients with osteoarthritis in the knees, hips and hands.

A Sad Day In Journalism

On Monday, January 6, 2014, an article was published in the Washington Post questioning the safety and efficacy of chiropractic manipulation of the spine.

The article highlights the most unfortunate risk that is attributed with chiropractic care.  However, more unfortunate, is the lack of perspective the author brings to light.  There have been many studies performed on both the efficacy and safety of chiropractic care.  Further, public opinion for chiropractic is one of the highest, if not the highest among the major medical providers.

However, if you look at the data of the “26 published cases” it shows correlation rather than causation in the stroke events.  The study omits important data such as familial and social risk factors, provider (DO, MD, or DC) who performed manipulation, and other medical risk factors like obesity, history of diabetes, and complications from pharmaceutics.  Further, if you put the number 26 in perspective with the over 20,000,000 people that are adjusted each year, it seems that the risk is statistically much more insignificant than the article leads you to believe.  Further, the article fails to highlight that all procedures have risks.  Like the unfortunate death of Jahi McMath from a tonsillectomy, no procedure is 100% without risk.  Moreover, if you look at the almost 1000 deaths attributed each year to acetaminophen (http://www.propublica.org/article/tylenol-mcneil-fda-behind-the-numbers) it seems foolish to call in to question the safety of chiropractic for treatment of neck pain when any person can walk into most any store and purchase a product that is more likely to cause harm.

I think the take home message of the article should be to encourage patients to be diligent in their pursuit of providers.  For example, in North Carolina, chiropractors are required to provide a list of possible side effects associated with chiropractic care.  In my office, we go one step further and discuss possible treatment outcomes with our patients so they have an idea of what to expect during the course of care.  In the end, I hope America will not just read the headlines, but take a deeper look at the safety, efficacy, and patient satisfaction chiropractic can provide for them.
